How to license my code?

Hello Everyone.

I'd like to clear something that I don't understand exactly. Let's say I have a source code and I'd like to publish it on the internet with a license. (GPL, MIT/BSD or something else)

Is it enough to put the license in a license.txt file next to my source files? Do I have to do anything else to properly license my code/app? Should I put the license to the top of each source files?
